School Health Form

This information will also be given to parents at the open houses but we are making it available here as well so that those of you who need to have the health/immunization form filled out can get that done prior and bring it in to the open house.  The health form does have to be within a year of expiration, so those of you who had yours done last August do need an updated form.  Your child may not require more vaccinations depending on what they had last, just an updated physical form.
